4～5年前のことですが80歳代前半の外来女性患者さんに私は、処方薬を少し整理しようと提案をしたことがあります。神経質な彼女は私の外来で5～6種類の処方を受けていました。彼女によくよく聞いてみると当科以外に整形外科、循環器内科、心療内科の3診療所を受診中とのことでした。処方されている薬剤の説明書を全医療機関分見せてもらって私は驚きました。何と全部で20数種類の薬剤が処方されていたのです。同じ薬効の薬が3ペアーありました。「これ全部飲んでるの？これ全部飲んだら死ぬよ」と思わず尋ねると、申し訳なさそうに「飲みやすいからこれとこれとこれを飲んでいる」との答え。循環器内科から処方されている彼女にとって一番必要だと思われる高価な薬は服用されてはいませんでした。
高齢者の五月雨受診が問題だと10年以上前から言われ続けてきました。月曜日は○○整形外科、火曜日は××眼科、—–、金曜日は☆☆内科と連日の外来受診、聞けば「友達と会えると寂しさが紛らわせるから」とのこと。これによって処方される薬はすぐに20～30種類にのぼり、その多くが捨てられている運命です。
捨てられるならまだ被害は少ないものの、服用すれば高齢者の腎機能・肝機能では薬害が発生する可能性は否定できません。ある腎臓専門医によれば糸球体腎炎などの原因疾患を持たない高齢者の腎機能の低下の一番の原因は多すぎる薬剤の服用だそうです。院外薬局でのお薬手帳による服薬指導が効果を発揮するはずですが、結果は上記の通りです。
これを管理・指導する役が後期高齢者医療制度に盛り込まれているかかりつけ医の意味です。高齢者の健康を管理し、薬剤の重複投与を防止し、ある時はゲートキーパーとして症状にあわせて必要があれば専門外来を紹介するこのかかりつけ医のどこがいけないのでしょうか？ 決してフリーアクセスが制限されるものではありません。
