私見ですが、高齢者に施行する手術など侵襲的医療は本来社会復帰を前提として行われるものですので、最終的に社会復帰が確認されたら追加して医療機関に収入があるような制度をとれないものかと考えます。前述のケースでは大腿骨頚部骨折の手術をして医療機関に対価としての診療報酬が支払われたあと、在宅復帰して一定期間に医療・介護費用が発生しなければ手術をした病院にボーナスが入るようにしてほしいと思うのです。そうなれば、病院は在宅復帰を最優先に早期にリハビリを行ってくれると思うからです。医師の目を担当の臓器から患者さんの全身管理へと向けさせるインセンティブが生じると思います。ボーナスは1000点（一万円）でも十分に効果が期待できると思いませんか。在院日数の短縮でその数十倍も医療費は節約できるはずですし、適正化される1.5兆円を引き合いに医療機関からボーナスの額を交渉する、健康を回復できた笑顔の対価を求める、そういう仕組み作りを医療関係者が提案し、現実化していくことが大切だと思います。
