今、療養病床に変革の波が押し寄せています。ご承知の通り、2011年度末には介護療養病棟の廃止が決定しています。医療療養病棟も2006年の改定から医療・ADL区分別の支払い方式が採用され、次第に病床数を減らしています。仮に政権が代わったとしても、民主党医療制度改革大綱によれば療養病床の30%に当たる11万床、一般病床は26万床もの削減が計画されています。
8月30日に投票日を迎える総選挙に向けて、各政党はマニフェストを出していますが、どれを見てもばらまき合戦の様相です。バラ色の給付と引き換えにだれがどう負担するのか、負担増の話には触れず、財源をどうするかはあいまいにしておいた方が選挙戦に有利だとの判断で、選挙戦になだれ込むようです。財政構造改革路線からはるかにかけ離れた政権運営がなされ、国(国民)の借金はさらに膨れ上がります。しかし、選挙が終わると現実的な問題として経済の低迷による税収不足から、歳出削減を求める声が高まってくるでしょう。われわれ医療や介護の業界は経済不況の数年後からその影響を受けると一般的に言われております。2年半後の2012年の医療と介護の同時改定はわれわれにとってさぞや厳しいものとなるでしょう。小泉構造改革路線は聖域なき行財政改革を進め、医療崩壊の原因を作ったと酷評されておりますが、その狙いは団塊の世代が高齢者の仲間入りを始める2012年までに、社会保障の伸びによる財政負担を将来に付け回さない仕組みを作ろうというものでした。現在の出生数の倍以上の2百数10万人が毎年毎年高齢者の仲間入りを始めます。年金を含めて国民への負担は増し、それによる医療・介護の単価の抑制は不可避です。2012年度は医療・介護業界厳冬時代の幕開けなのです。
