急性期病院を60歳あるいは65歳で定年になって、一枚の医師免許証を頼りに療養病床へ勤めを変えることは、半ば当たり前のように行われて参りました。そればかりか医師不足で悩む地方の療養病床では定年後の医師はまだまだ引く手あまたです。医療密度の濃い病棟は慢性期医療では数少なく、今を勝負しないと患者さんが命を落としてしまう急性期病床と比べ、患者さんは明日まで待っていただけます。ゆっくりと調べ物をして、あるいは学んでからでもOK。精神的な余裕が違います。
複線型の教育、スキルアップの必要性を書きましたが、私自身も15年ほど前、ケアミックス型の福岡の白十字病院で療養病棟（当時は介護力強化型病棟と呼んでいたと思います）で7～8人の患者さんを担当していた折、高齢者に対する臨床力のなさを痛感し、戸惑っておりました。当時の療養病棟は過半数の患者さんが経鼻経管栄養で、関節は拘縮した、いわゆる寝かせきりの病棟でした。褥瘡や排尿障害、誤嚥性肺炎、高齢者特有の鬱病や不眠、認知症に対する対処が課題でした。これは現在の療養病棟でも同様と思われます。しかしながら、急性期病棟から転向してきた医師でこれら各領域に対応できる医師はいったいどれだけいるのでしょうか。これらを履修する機会は医学部の教育の中に、ほんの一瞬、断片的に出ては参りますが、高齢者を対象とした系統的な講義ではないために、自信を持って診療に当たれるレベルには達せません。私の知る限り、全国の大学病院にはただの1床も療養病床はなく、学生や研修医が慢性期医療の現場を知る機会はありません。解決しなければならない課題が山のように山積しているのに、慢性期医療を大学が研鑽の場として認めていない以上、系統的な講義はもちろん、新しい知見や技術の開発は望めません。老年科はいくつかの大学にあるものの、研究の対象が高齢者というだけで、高齢者特有の生理や機能を教室員すべてに“慢性期医療のプロ”として教育されている大学を私は知りません。急性期病床として現在稼働している病床数をその数で上回り、長い時間とお金をかけて運営される療養病床が、いまのまま振り向かれない存在でいいのでしょうか。「そんなことは民間でやればよい」「大学でわざわざ研究することではない」と相手にされないままでよいのでしょうか。
数年前、ある大学病院の責任のある立場の教授に、「ぜひ大学に20床程度でいいから療養病床を作ってください。救命救急部で人工呼吸器に繋がれて長期入院している患者さんや病状が安定しないため退院が遅れている患者さんを集めてそのケアを学問として評価し、それを全国に発信してください」と提案したことがあります。無理なお願いと思ってはおりましたが、やはり体よく断られました。
