5、6年前、われわれの病院で医療事故が起こりました。下部胆管狭窄の女性患者さんに対してERCP 、乳頭拡張術の術中に起こったもので、操作により膵管を傷つけ急性膵炎を起こしました。CT検査で腹水が貯留し、重症膵炎の状態でした。患者さんのご家族の2人の息子さんたちは遠方にお住まいのため、検査の事前の説明は消化器内科医が患者さんご本人にのみ行っていました。ERCP,EPBDの手技や合併症を含めて詳しく図示しながら説明していましたが、息子さん達には伝達されていませんでした。事故の後、後腹膜ドレナージの必要性から若手の外科医も治療に加わりました。急遽息子さんたちに来院していただき、医師団から状況説明がなされましたが、ご家族からは強いご不満と厳しい指摘の声があがりました。病状は一進一退を繰り返し、一時はショック状態に陥りましたが、幸いにも一命を取り留めました。遠方から通ってこられるご家族には大変なご不自由とご負担をおかけいたしましたが、一方で、当院の医師も週末は家族説明のため、休日出勤を余儀なくされました。病棟看護師に対するご不満も多く、例えば「24時間看護のはずなのに、何時も母に看護師がついていない、だから痛みがとれていない」などたびたびお叱りを受けました。入院は10か月に及びましたが、無事に退院を迎えたスタッフは、患者さんに申し訳なく思うのと同時に、合併症を医療ミスと誤解されてしまった後では努力してもコミュニケーションはとれず、説明の大切さをいやというほど知らされました。スタッフも大変なストレスだったと思いますが、私にとって最もショックだったのは、それから暫くして治療にあたったその30代の前途有望な外科医が、燃え尽きてわが病院を辞めてしまった事でした。理事長に就任して4年、大きな試練の一つでした。

Those concerned with privacy will be relieved to know you can prevent the public from seeing your personal listening habits if you so choose.

If this changes, then it will somewhat negate this advantage for the Zune, but the 10 songs per month will still be a big plus in Zune Pass’ favor.

Hands down, Apple’s app store wins by a mile. It’s a huge selection of all sorts of apps vs a rather sad selection of a handful for Zune. Microsoft has plans, especially in the realm of games, but I’m not sure I’d want to bet on the future if this aspect is important to you. The iPod is a much better choice in that case.
