現在、地方における医師不足が深刻です。佐世保中央病院も例外ではなく、医師の過度な負担を軽減するために、医師でなくでも代行できる部分をサポートしたいと考え、法人を挙げて取り組んで参りました。既に着手できたものとして、医療秘書と説明支援ナースが挙げられます。医療秘書は、すでに多忙なDr.をサポートする体制には欠かせない存在です。加えて説明支援ナースは、検査や手術の方法、予想される経過や起こりうる合併症の種類とその発生頻度、その対処法などを、患者さんや家族に詳しく、優しく、そして繰り返し理解させてくれる存在です。次に多職種協働で行ってきたことは、患者さんや家族への支援です。まず、自覚症状に乏しい糖尿病、高血圧、高脂血症など生活習慣病患者を心筋梗塞や脳血管障害に至らせないために、根気強い教育的支援・指導が必要です。すでに始まったメタボ対策をはじめとする予防医学への取り組みが期待できる施設は、佐世保地区においては、種々の療養指導士が実働している当院のみだと思われます。さて、これからは、これらをさらに一歩進めて、看護師の専門性を発揮できる場の提供を行いたいと考えています。それは、『看護師による看護外来』という構想です。前述しました糖尿病療養指導士、リウマチ膠原病療養指導士、説明支援ナースに加えて、緩和ケア、感染制御、皮膚ケア（WOC）、NSTなどの認定看護師や法人内認定看護師が各科の枠を超えて、患者さんのために活躍できるような『看護外来』をつくりたいと考えています。専門的な看護師の立場からの評価と指導は患者さんにとって強い精神的サポートや当院への信頼に繋がると確信しております。患者さんが病気を理解し、自ら戦うには、時には強く、時には優しい頼りになる身近な相談者が必要だと思うからです。リンパ管マッサージや指導を行うリンパ浮腫支援相談室、ブレストケアナースが中心となって脱毛や精神的支援も併せて行う乳がん支援相談室、などなど様々な取り組みが期待されます。医師としてやりがいのある夢を現実のものにしようではありませんか。
